Registration and issuance of copies of birth, marriage and death certificates 

Registration of Birth

 

  • A birth is registered by the Registrar of Births and Deaths of the place where the birth occurred.
  • If the birth occurred in a hospital, the Registrar of the place or the person assigned to the hospital must be notified for the registration of the birth.
  • Persons eligible to be notified for birth registration,

                 Father               

                 Mother                 

                  People who were present at the time of the birth, people in charge of the child

                Hospital medical officer

  • If the parents are unmarried, both parents must go to the Registrar of Births and Deaths at the hospital where the birth took place to enter the father's information.

  • The registration form CR01 required for notification of birth is available from the Registrar of Births and Deaths and can also be downloaded from the website. 
  • Required documents  for birth registration

  1. Duly completed registration form CR01
  2. Report issued by the hospital confirming that the birth took place
  3. A photocopy of the parents' marriage certificate
  4. A photocopy of the parents' birth certificates
  5. Original and photocopy of the national identity card
